A Lump Sum Payroll Request Form will be enclosed with your March 22 paycheck or direct deposit stub. The form is also available on the Payroll Office website at Hudson Valley Lump Sum Payrol Request Form.
The Lump Sum pay will be issued on May 31, 2013.
If interested, the form should be signed and returned to the Payroll Office by Friday, May 3, 2013.
