A Lump Sum Payroll Request Form will be enclosed with your March 23rd paycheck or direct deposit stub. The form is also available on the Payroll Office website at:
The Lump Sum pay will be issued on June 1, 2012.
If interested, the form should be signed and returned to the Payroll Office by Friday, May 4, 2012.
