As announced in November, the electronic Payroll Authorization is available for department use and can be accessed from the Human Resources Forms and Documentation page.
The new form should be used for all changes in employment and salary, with the exception of new hires, resignations or retirements.
Please do not use the PDF or paper forms from this point on.
