Protect your self against the flu!
An on-site flu vaccine clinic is offered by Rite Aid on Wednesday, Sept. 12 from 10 a.m. to 2 p.m.
Rite Aid offers direct insurance carrier billing provided flu immunization is a covered service. To facilitate the process, Rite Aid requests pre-regisration and submission of a copy of your current insurance card when scheduling your appointment. Information needed to coordinate the benefits: BIN #/ PCN #/ ID # and Group.
Self pay option: $39.99
Please contact Health Services at (518) 629-7468 to pre-register for the Flu Clinic . You will need to provide insurance information and obtain a copy of the screening questionnaire and consent form. Appointments will be available every 15 minutes.
The next Flu Clinic will be held on Monday, Oct. 15 from 11 a.m. to 3 p.m.
