As announced in Friday’s article, New Electronic Form System to Debut with Payroll Authorization on Monday, Nov. 15, the new electronic Payroll Authorization is now available for department use.
The link to the new form has been posted to the Human Resources Forms and Documentation page.
The new form should be used for all changes in employment and salary, with the exception of new hires, resignations or retirements.
Please do not use the PDF or paper forms from this point on.
