Student Vaccine Deadline is Tomorrow, Sept. 30

 

As you know, SUNY and New York State have mandated that all students taking on-campus classes or accessing on-campus services this semester will need to be vaccinated against COVID-19 to continue coming to campus. The deadline for students to receive at least one dose a COVID-19 vaccine and submit proof to Health Services is tomorrow, Sept. 30.

If you have already been vaccinated, please submit your proof of vaccination to Health Services via the Pooled Testing Exemption webpage. You will be prompted to log in with your college username and password to submit your proof of vaccination.

If you have not been vaccinated, there is still time to receive your first dose before the deadline. Find a vaccination site close to you at www.suny.edu/knowyourvax. Once you have received your first dose, follow the steps above to submit your proof of vaccination by the end of the day tomorrow, Sept. 30.

If you cannot get vaccinated for religious or medical reasons, you must fill out a Request For Waiver From Immunization (PDF) form and submit it to Health Services by the end of the day tomorrow, Sept. 30.

Those who have not submitted proof of receiving at least one dose a COVID-19 vaccine or received a medical or religious exemption by the end of the day tomorrow, Sept. 30, will be administratively withdrawn from on-campus classes without a refund, and will not be allowed on campus for the remainder of the semester.
