Required Vaccines<\/strong>:<\/p>\n Most students can request their records from their high school or prior college, doctor\u2019s office, or place of employment. Students may also obtain MMR titers (bloodwork that tests for antibodies) from their local doctor\u2019s office or urgent care if they do not have access to their MMR records<\/p>\n SUNY COVID-19 Vaccine Mandate<\/strong> If you need assistance with the COVID-19 vaccination record, please call Health Services at (518) 629-7468 or email healthservice@hvcc.edu<\/a>.<\/p>\n Q. Additional Student Health Forms Required for Some Students<\/strong><\/p>\n For Health Science students participating in clinical placements:<\/p>\n For Student Athletes<\/p>\n Completed forms should be emailed to\u00a0 healthservice@hvcc.edu<\/a>.<\/p>\n If you are transferring to another school and need immunization records, please submit an Authorization for Disclosure Form (PDF)<\/a> to healthservice@hvcc.edu<\/a>. We will gladly send your records to your new school.<\/p>\n Weekly pooled surveillance testing is available to all students and employees, however, weekly testing is required for all unvaccinated or partially vaccinated students, including those with an approved exemption who are on campus.<\/p>\n Students may also meet the weekly testing requirement by submitting results of an at-home test to Health Services using the\u00a0 Documentation of COVID-19 Test Results form<\/a>.
