\n— Contact information (e-mail, phone and address)
\n— Name of act
\n— Number of performers and their instruments; any Vocals
\n(or if you would like to play with the House Band)
\n— Indicate if act is Spoken Word, Comedy or Dance (not to exceed 3 minutes in length)<\/p>\n

To secure any remaining spots, please be present in the Maureen Stapleton Theatre lobby at 1 p.m. on Friday, April 4.\u00a0\u00a0The Open Mic schedule will be posted in the lobby at 1:30.\u00a0\u00a0All Open Mic performers must be present at that time to confirm their participation.
