A Lump Sum Payroll Request Form was enclosed with your March 25 paycheck or direct deposit stub. The form is also available on the Payroll Office’s Web site at:

For those who work a 170-day work year, the Lump Sum pay will be issued on June 3, 2011. If interested, the form should be signed and returned to the Payroll Office by April 21, 2011.
For those who work a 195-day work year, the Lump Sum pay will be issued on July 15, 2011. If interested, the form should be signed and returned to the Payroll Office by June 3, 2011.
