A Lump Sum Payroll Request Form was enclosed with your March 25 paycheck or direct deposit stub. The form is also available on the Payroll Office website at:
The Lump Sum pay will be issued on June 3, 2011.
If interested, the form should be signed and returned to the Payroll Office by May 6, 2011.
